We’re sorry an incident has occurred, and we understand this is probably a stressful situation. We’re here to help. Priority #1 is to get the situation under control, and any injuries properly treated. Take advantage of any day of loss services you have at your disposal including Nurse Triage, Occupational Medical Centers, and an Accident Response team.
Once that is taken care of, it is important to get us as much information as you can as soon as possible. To initially report an incident, please use one of the forms below. Do NOT report by email. Once the initial report is complete, follow up with supporting documentation as you receive it. Witness reports, supervisor reports, photos, voice recordings, etc. Please upload them to the claims file in your customer portal, or if need be email them to the assigned Claims Advocate.
